If I understand correctly, it's proper to say:

Tá mé ag léamh an leabhair = I am reading the book

Is it ever the case that the nominative "leabhar" is used in that sentence, in everyday speech, even though it's incorrect?

Well, in everyday speech you'd hear any kind of mistakes, depending on who is speaking ! Learners, non-native speakers, people who've been raised up through Irish by learners, young Gaeltacht speakers, older Gaeltacht speakers...

I guess many speakers, in all Gaeltachtaí (I guess) may use the nominative in your sentence. And of course, non-Gaeltacht speakers may use it even more.

By the say, in Donegal "leabhar" is feminine so people (those who'd use the genitive, of course) would say
"Tá mé 'léamh na leabhaire" or "...na leabhra".
