Peter,
sorry to be a party pooper, but how exactly is one to fill it out? Cut +paste to Word and send as an attachment?

Remember, one is asking a favour of the participant -do everything but fill out the form for them.

Only last week I was asked to do a questionaire that required me to collate the answers at the back when finished so as to make it easier for the psychology student, as if it were my job. I doubt few bothered

yep.

Any kind of answer in whatever form is more than welcome, I gather. You can copy the form into the letter and write to every item whatever you like, as much as you can or want.
